Job summary

YWCA of Hawai'i Island seeks a dedicated Family Support Specialist to provide home-based services to families under the Clinical Supervisor. You will work with a collaborative team to deliver culturally sensitive support and advocacy per Healthy Families Program policies.

You will build trusting relationships, guide parenting and child development, refer families to community resources, document observations, and participate in ongoing trainings. Some evenings or weekend work may be required.

Responsibilities

  • Provide home-based services to families in accordance with Healthy Families Program policies.
  • Schedule and conduct home visits, ensuring services are culturally sensitive and meet family needs.
  • Foster positive, trusting relationships with families, offering guidance on parenting and child development.
  • Collaborate with a multidisciplinary team to address identified risk factors and family needs.
  • Communicate observations related to child safety, mental health, domestic violence, and substances to team members.
  • Refer families to relevant community resources (e.g., DHS, WIC) and support transitions to other services.
  • Participate in staff meetings and Family Goal Plan reviews; track progress and document updates.
  • Maintain confidential records per program standards and complete required documentation.
  • Attend required trainings and duties; some evening and weekend work may be required.

Position Overview

As a Family Support Specialist, you will work under the general direction of the Clinical Supervisor to provide home-based support services to families, fostering positive relationships and healthy development. You’ll be part of a collaborative team, working closely with other professionals to ensure families receive comprehensive, culturally sensitive services tailored to their needs.

Essential Duties and Responsibilities
  • Uphold and further the mission of the YWCA of Hawaii Island, which is dedicated to eliminating racism, empowering women, and promoting peace, justice, freedom, and dignity for all.
  • Provide supportive, home-based services to families in accordance with Healthy Families Program policies and procedures.
  • Contact families to schedule and conduct home visits, ensuring services are culturally sensitive and meet individual family needs.
  • Foster positive, trusting relationships with families, offering emotional support and guidance on parenting, child development, and overall family well-being.
  • Collaborate regularly with a multidisciplinary team, consulting on family needs and developing strategies to address identified risk factors.
  • Communicate observations related to child safety, mental health, domestic violence, and substance abuse to the appropriate team members or agencies.
  • Refer families to relevant community resources and programs, such as DHS or WIC, and support transitions to other services when needed.
  • Participate actively in staff meetings, child team discussions, and Family Goal Plan (FGP) reviews and meetings.
  • Help implement and track progress on Family Goal Plans, including documenting updates and strategies.
  • Conduct approved developmental screens and assessments with guidance from the professional team.
  • Maintain timely, accurate, and confidential records in accordance with program standards, utilizing designated tools and systems.
  • Complete required organizational, contract, and program documentation.
  • Attend all required trainings, meetings, and professional development sessions.
  • Perform other duties as assigned, including occasional evening and weekend work.
