Job summary

Southwest Human Development is Arizona’s largest nonprofit focused on early childhood development, with a mission to support young children and their caregivers. We seek a Parents Partners Plus Specialist to triage referrals, connect families to programs, and maintain accurate data while building strong community partnerships.

This role may include evenings and weekends and requires a valid AZ driver’s license and clear background checks.

Qualifications

• 1 year of experience in a family support or case management role.
• Bachelor’s degree in Early Childhood Education or related field.
• Insured & registered vehicle and AZ driver’s license.
• Valid Arizona Fingerprint Clearance Card or qualify for Level 1.
• DES Criminal Affidavit required.
• AZ DHS Criminal History Affidavit required.
• Bilingual in English/Spanish preferred.

Responsibilities

• Coordinate and process referrals daily, timely entry and assignment to providers.
• Conduct initial family screenings and follow-up communications per timelines.
• Maintain knowledge of Alliance member programs to match families to services.
• Enter and monitor data and documentation per program requirements.
• Manage referral waitlists and coordinate out-of-area referrals.
• Follow up with providers to confirm service engagement and outcomes.
• Distribute community resource and referral information.
• Build relationships with hospitals and community partners.
• Support outreach, health fairs, and events, including evenings/weekends.
• Assist with marketing and social media content.
• Prepare reports and support Alliance activities and funding.
