Job summary

West Texas Centers is seeking a dedicated Family Partner to mentor parents of children in our care under WTCMHMR. The role focuses on peer support, advocacy, and close collaboration with the Case Manager to address resources and needs.

You will co-lead the Parent Support Group, attend CRCG meetings as needed, and maintain timely progress notes and logs. The position emphasizes confidentiality and compliance with Medicaid/Medicare policies.

Responsibilities

• Will provide mentoring to parents of children in services with WTCMHMR.
• Will work closely with the Case Manager.
• Will co-lead the Parent Support Group at the local site with the Case Manager.
• Will become an advocate for the parent with WTCMHMR as needed.
• Will attend local CRCG meetings.
• Will complete progress notes within 24 hours and give to the Case Manager.
• Will keep a log of time spent with each parent, mileage to and from the meeting place and the topic covered.
• This log will be kept at the local site; however, a copy of it will be submitted monthly to the Director of MH Services.
• Confidentiality.
• Reports possible abuse/neglect.
• Employee will comply with all WTC policies & procedures related to Corporate Compliance involving Medicaid, Medicare and/or other pay sources.
• Must take, pass and maintain all mandated training components.
