Job summary

The Whole Child Mental Health in Whittier, CA is seeking a dedicated Parent Partner to mentor parents involved with the system and to serve as the parents’ voice. You will provide advocacy, support and referral services to clients on your caseload, helping families access community resources and services.

Responsibilities include coordinating with a multi-disciplinary team, documenting activities, and guiding parents in accessing resources such as DCFS, Probation or Mental Health systems.

Qualifications

• Provide advocacy, support and referral service to clients assigned to their caseload.
• Assist families in linking to community services within the scope of EPI-LA.
• Provide in-home support services demonstrating family-centered, help-give skills and addressing activities within the Client Care/Coordination Plan
• Collaborate with a multi-disciplinary team to address needs of client/families and facilitate community services linkage
• Document all activities in accordance with Medi-Cal regulations and protocol of EPI-LA Services
• Act as liaison between EPI-LA staff, community providers, families, and children, facilitating an alliance to The Whole Child mission
• Participate in identifying clients’ needs and triggers to escalation and barriers; make recommendations to supervisors and treatment team
• Provide teaching and guidance to parents and caregivers to help them manage resources (e.g., DCFS, Probation or Mental Health systems)
• Provide individualized treatment planning for clients and clients access and keep appointments
• Develop and maintain positive relationships with parents and act as a liaison between client, agency and community resources

Responsibilities

• Assist in providing field contacts and ensure clients’ access to services
• Assist clients and caretakers in linking to community services within the scope of EPI-LA
• Provide in-home support services that demonstrate family-centered, help-give skills
• Collaborate with a multi-disciplinary team to address needs and facilitate services linkage
• Document all activities per Medi-Cal regulations and EPI-LA protocols
• Act as liaison between EPI-LA staff, community providers, families, and children
• Identify clients’ needs and triggers and escalate barriers; report to supervisors
• Educate parents to access resources and develop problem-solving skills
• Facilitate interventions and plan program events
• Complete paperwork timely and thoroughly
• Accompany parents to important meetings like court proceedings
• Other related duties as assigned
