The Buckeye Ranch in Norwood, OH is seeking a Kinship & Adoption Navigator to support kinship caregivers and adoptive parents. You will meet weekly with the OhioKAN Regional Coach to deliver services with fidelity and coordinate access to needed supports.

Responsibilities include assessing family needs, developing individualized care plans, maintaining records, and hosting community events. Travel is up to 10% as part of regional outreach.

Qualifikationen

• Associate's or Bachelor's degree in Social Work, Public Health, Education, or related field (preferred).
• Direct experience working with families and/or professional case management is preferred.
• Proficient with necessary computer applications, including Microsoft Outlook and case databases.
• Active driver's license and proof of valid auto insurance; driving is an essential function.

• Partner with families to assess needs and set goals; conduct detailed assessments, develop care plans, and advocate for services.
• Build regional resource lists and partner networks to create service directories and robust referral pipelines.
• Maintain case records and ensure fidelity; manage family records and participate in weekly consultations with Regional Coach.
