Job summary

Children’s Home Network is seeking a Family Navigator to support families in Hillsborough County, delivering navigation services, coordinating care, connecting families with community resources, and documenting services to support prevention of child abuse and neglect.

You will work with parents and caregivers, participate in Family Team Conferences, and collaborate with a multidisciplinary team to develop and monitor family plans, ensuring appropriate supports and referrals.

Qualifications

• Bachelor’s Degree required with at least one year of experience working with children and families.
• Understanding of issues related to parenting and child development.

Responsibilities

• Help families obtain community resources and training.
• Provide parenting support, education, and care coordination to at-risk caregivers.
• Monitor caregiver and child needs and help implement intervention plans.
• Participate in Family Team Conferences.
• Collaborate with a multidisciplinary team to discuss progress and supports.
• Complete timely documentation according to program requirements.
