Job summary

Communitas, Inc. in Wakefield, MA is seeking a Family Support Coordinator to deliver home-based, family-centered services for individuals with developmental disabilities. The role focuses on connecting families with supports, benefits, and community resources while collaborating with DDS and DCF agencies.

The position requires a Bachelor's in a related field, valid driver's license, and commitment to empowerment. Full-time, 40 hours weekly with competitive pay and benefits are offered.

Qualifications

• Bachelor's Degree in Social Work, Education, Human Services or a related field is required.
• Up to two years of family support case management experience preferred.
• A passion and dedication to supporting our people is a must.
• Valid Driver’s license, reliable transportation, and acceptable driving record.
• Must pass company background and reference checks.

Responsibilities

• Inform the family and assist them in accessing supports and services that exist within the community.
• Assist families in obtaining all state and federal entitlement benefits such as SSI, CommonHealth clinical/medical supports, and behavioral supports.
• Understand Massachusetts Special Education Laws.
• Assist families in working with collaterals and other agencies.
• Supervise support staff working in families homes.
• Attend Intake and Review Meetings at DCF and develop Individualized Family Treatment Plans.
• Document progress and prepare reports for DCF review meetings.
