Job summary

PathWays in New Hampshire is seeking a Family Support Coordinator to serve families across Sullivan and Lower Grafton County. You will connect families to resources, help with transitions from school to adult services, and document outcomes as part of a collaborative team.

What You'll Do

As a Family Support Coordinator, you will be the ultimate bridge to vital resources, community connection, and advocacy:

  • Advocate & Empower: Connect families to state and community resources while maximizing their control over their own services.
  • Guide Transitions: Help individuals smoothly plan and navigate the massive milestone of transitioning from school to adult services.
  • Navigate Legal & Financial Pathways: Assist with budget administration and facilitate important processes like Guardianship and Power of Attorney.
  • Collaborate & Document: Facilitate team planning meetings and maintain meticulous documentation alongside a passionate, friendly team.
What We’re Looking For

We need a flexible, creative, and non-judgmental professional who genuinely loves serving diverse families.

  • Education: A Bachelor’s degree is highly preferred, but extensive case management experience will absolutely be considered instead.
  • Experience: At least 2 years in human services, with a minimum of 1 year in family support.Experience with developmental disabilities is a huge plus!
  • Skills: Excellent communication, strong organization, intermediate MS Office proficiency, and a willingness to learn new programs.
  • Travel: Reliable transportation for regional/statewide home visits and meetings.
