Job summary

PathWays NH seeks a compassionate Family Support Case Manager to connect families with resources, support transitions from school to adult services, and assist with guardianship and financial pathways. The role requires strong communication, organization, and willingness to learn new programs.

A hybrid work arrangement may be available after 90 days, with regional home visits and a collaborative team environment.

Qualifications

• Bachelor’s degree is highly preferred, but extensive case management experience will absolutely be considered instead.
• At least 2 years in human services, with a minimum of 1 year in family support. Experience with developmental disabilities is a huge plus!
• Excellent communication, strong organization, intermediate MS Office proficiency, and a willingness to learn new programs.
• Travel: Reliable transportation for regional/statewide home visits and meetings.

Responsibilities

• Advocate & Empower: Connect families to state and community resources while maximizing their control over their own services.
• Guide Transitions: Help individuals smoothly plan and navigate the massive milestone of transitioning from school to adult services.
• Navigate Legal & Financial Pathways: Assist with budget administration and facilitate important processes like Guardianship and Power of Attorney.
• Collaborate & Document: Facilitate team planning meetings and maintain meticulous documentation alongside a passionate, friendly team.
