Job summary

ServiceNet in Greenfield, MA is seeking a Family Support Autism Case Manager to join our Family Empowerment team. You will provide training, information, and referrals to families and help educate the community about disabilities and autism.

The role involves developing support plans, connecting families to resources, coordinating services, and facilitating community-based activities. A high school diploma and at least one year of related experience are required.

Qualifications

• High School Diploma / GED required.
• Over 1-year direct experience in similar human services agency case work with families or individuals with disabilities.
• Broad knowledge of disability support services.
• Valid driver's license and reliable transportation/vehicle required.

Responsibilities

• Providing individualized support to families.

• Maintain contact and ongoing support as needed.

Family Support Autism Case Manager Full-Time

Location: Greenfield, MA with travel throughout Hampshire and Franklin Counties as needed.

Salary

$20.00/hour

In Family Empowerment, our Family Support Case Managers are a key resource to families who have a member with a disability. You will provide training, needed information, family support contacts and referrals to other agencies and supports. On a wider level, you will help educate the community about disabilities and/or autism and how to help better support families, as well as facilitating social and recreational activities for people of all ages. You will help families get the best results from funds available by administration of stipends and all documentation.
