Job summary

Qualifications

Responsibilities

• Maintain a caseload of 12 children.
• Conduct initial intake to assess families and obtain documents.
• Maintain regular contact with families and service providers.
• Develop and present service plans and goals.
• Create comprehensive referrals to providers.
• Attend unit meetings and agency functions.
• Provide transportation as needed per policy.
• Ensure HIPPA and PINS regulations compliance.
