Job summary

Hope For Youth, Inc. is seeking a Family Peer Advocate to provide rehabilitative support to families and youths, work within an interdisciplinary team, and help connect families to community resources.

Duties include direct family contact, documentation, and participation in team meetings. Qualifying candidates will have lived experience as a parent or caregiver of a child with mental health needs, a credential or willingness to obtain it, and proficiency with Microsoft Office and EHR systems.

Qualifications

• Family Peer Advocate Credential or willingness to obtain the credential within a set timeframe.
• Lived experience as a parent or caregiver to a child with mental health diagnosis.
• Computer proficiency, including Microsoft Office Suite, and experience working with EHRs.
• Highschool Diploma or GED.
• Valid NYS driver’s license in good standing.
• Physical Requirements: read/write reports and log entries, walking/standing/sitting for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, daily driving own vehicle (mileage reimbursement available).

Responsibilities

• Function within the policies and procedures as outlined in program practice guide and employee manual.
• Adhere to state regulations and applicable code of conduct.
• Work within an interdisciplinary team to assist client in their overall functioning and achieve desired outcomes.
• Provide rehabilitative support services through an array of formal and non-formal services to the parent, family member or caregiver for the benefit of the child/youth.
• Assist families with developing community connections and naturals supports to enhance the quality of life by integration and supports for families in their own communities, as well as helping the family to rediscover and reconnect to natural supports already present in their lives
• Helping the family learn and practice strategies to support their child's positive behavior, assisting the family to implement strategies by clinicians, providing individual or group parent skill development related to the behavioral health needs of the child (i.e., training on special needs parenting skills).
• Attend trainings and professional development as assigned
• Attend daily team meetings and family meetings as assigned
• Apply agency-identified Evidence Based Interventions best suited to meet the client's needs
• Provide transportation for client to their service location as necessary
• Perform other resulting and necessary work to meet the needs and requirements of the clients, the program, Hope For Youth, oversight agency and any other regulatory obligations
