Job summary

AWARE in Missoula, MT is seeking a Family Support Specialist to join our compassionate team. You will maintain an individual caseload, support families to enhance independence, and work with clinical staff in home and community settings.

The ideal candidate has a BA/BS in Human Services, Psychology, or Social Work and at least one year of direct experience; strong communication, cultural sensitivity, and flexibility to work evenings/weekends are required.

Qualifications

• Knowledge of human behavior and performance.
• BA/BS in Human Services, Psychology, Social Work, or related field preferred; 1 year direct work experience.
• Valid Montana Driver’s License and clean background check may be required.

Responsibilities

• Maintains an individual caseload established with the designated Service Administrator and Service Director.
• Provides support to individuals and their families to manage in the least restrictive setting and to enhance self-sufficiency skills.
• Works closely with AWARE’s Clinical Professionals in providing services as assigned and developed.
• Assists the person’s entry into service and monitors needed services throughout the service system.
• Engages family to identify strengths, needs and goals.
• Develops, implements, and updates the Individualized Treatment Plan (ITP) every 90 days.
