Job summary

Children's Home Society of Florida is seeking a Dependency Family Support Worker (FSW) to help deliver case management services for children and families. You will assist with documentation, transportation, and linkage to services while fostering a supportive home environment.

Qualifications

• High school diploma or GED required.
• Florida Driver’s License within 30 days from hire with daily access to a reliable and insured vehicle.
• Two years of experience working with children and families, required.
• Experience in a social services organization, preferred.
• Knowledge of community resources and networks to enhance client experiences and results.
• General confidentiality requirements and ethical standards in handling sensitive client-related data.

Responsibilities

• Assist with documentation, transportation, and delivery of services.
• Assist with development and maintenance of client records and program documentation (initial assessments, goal planning, progress notes termination summaries, etc.).
• Enter necessary information into required software system(s) in a timely and accurate manner.
• Assist with delivery of case management support services (information, assessment, referral and linkages).
• Directly transport or arrange for transportation of clients to medical appointments, counseling, shopping, etc.
• Supervise children during scheduled visits, in-home services, visitation or in other situations as directed.
• Participate in weekly client staff meetings, as requested.
• Compliance with CHS policies and procedures and code of conduct.
