Job summary

United Way of Central Alabama is seeking a Family Stability Case Manager to deliver comprehensive case management and housing counseling services. You will conduct outreach, assess needs, and develop individualized plans to improve housing stability and financial well-being for families.

The role requires collaboration with community partners, managing a caseload, and ensuring progress toward plan objectives with ongoing documentation and follow-up.

Responsibilities

  • Perform outreach and recruitment activities with residents and community stakeholders.
  • Manage assigned caseload from Family Stability referrals and provide quality case management and housing counseling services.
  • Develop budgets and financial plans for participants in the Family Stability program.
  • Conduct assessments on housing, money management and education attainment.
  • Develop plans to help families achieve housing and financial stability and keep children enrolled in school.
  • Obtain family endorsement and releases for outreach, advocacy and case management.
  • Provide progressive case management and monitor outcomes for at least three months.
  • Connect families with community resources and follow up on referrals.

Job Summary

Responsible for all case management services of assigned clients through the Family Stability Services program. Conducts outreach activities designed to promote program visibility, support community understanding of available services and increase both direct participation and referrals.

Responsibilities

  • Perform a variety of outreach and recruitment activities through direct interaction with residents and community stakeholders.
  • Manage assigned caseload from Family Stability referrals and provide quality case management and housing counseling services, including development of budgets and financial plans for all participants in the Family Stability program.
  • Conduct assessments to include at a minimum housing, money management and educational attainment.
  • Develop plans to help families achieve housing and financial stability and help children maintain school enrollment.
  • Obtain the family’s endorsement of and participation in the plan along with requisite releases, authorizations to provide outreach, advocacy and/or case management.
  • Provide progressive case management and monitor outcomes with family until family achieves case plan objectives and for a minimum of three months.
  • Connect families with needed and available community resources.
  • Follow-up with clients and community resources as appropriate to document use/success of referral.
  • Adhere to program guidelines and prescribed service delivery as identified by funding sources.
  • Maintain client records and input ongoing client notes into program’s case management software, including intake documents, releases and ongoing progress towards goals.
  • Maintain s accurate records in accordance with professional, agency and applicable grant standards.
  • Refer clients to financial education classesParticipate in regular staff meetings, training programs, and supervisory sessions.
  • Adhere to agency polic i es, procedures and the professional code of ethics.
  • Participate in Community Initiatives’ Continuous Quality Improvement Plan.

Education/Experience

Bachelors degree in SW, Counseling, or social service related degree

Experience in counseling/case managing high risk populations

Excellent computer skills to include Microsoft and database

Working Conditions:

Normal office environment

Frequent work in the community – including travel to and from sites

Some after-hours and/or weekend work may be required

Occasional out of town travel

Requirements:

Must meet and maintain eligibility requirements to be insured by UWCA’s insurance

Reliable transportation

Proof of automobile insurance commensurate with State law

Background checks to include but not limited to criminal record, drug screen
