Job summary

Wellstone in Cullman, AL is seeking an experienced Case Manager to provide outpatient services for seriously mentally ill clients, support the therapeutic team, and guide clients toward stable outpatient treatment. This full-time role requires a Bachelor's in social sciences, up to 80% travel, and strong communication skills.

You will help with living skills, medication access, and coordination with residential care providers.

Responsibilities

  • Provide outpatient case management services to the seriously mentally ill population.
  • Provide direct services to severely mentally ill clients and connect them with continuum of care.
  • Provide money-management training to meet client needs for psychiatric stability.
  • Provide follow-up progress notes to treatment team members.
  • Evaluate residential stability and assist in securing stable environments.
  • Provide basic living skills training to prepare clients for less intensive care.
  • Assist clients with obtaining medication and monitor adherence.
  • Maintain documentation and electronic charting for clients.
  • Coordinate transportation and referrals with collateral services.
  • Develop relations with Residential Care managers to meet client needs.
  • Develop and implement Individual Service Plans with goals and resources.
  • Review progress and revise plans at set intervals.
  • Address crisis situations and coordinate with other caregivers.
  • Support families of mentally ill clients with education and guidance.
  • Complete other duties as assigned.

Job Description: The Case Manager is responsible for providing resources for care, assisting the therapeutic team, and encouraging healthy choices as clients transition to outpatient therapy and treatment.


What you'll be doing:


  • Provide outpatient case management services to the seriously mentally ill population.

  • Provide direct services to severely mentally ill clients to address needs which are unmet as well as information about continuum of care.

  • Provide training on money management as it relates to meeting needs of the client to obtain and maintain psychiatric stability residential setting.

  • Provide follow-up progress as per need of client to treatment team members.

  • Evaluate client’s residential stability and assist those clients who are in need of more stable environment.

  • Provide Basic Living Skills training when necessary as it relates to client's immediate needs while preparing client for less intensive level of care within the community.

  • Assist clients with obtaining medication.

  • Monitor medication compliance, maintain awareness of common side effects, and ensure client is taking medication as directed on a regular basis.

  • Provide traditional case management services as well as collateral services such as transportation to appointments, referral agencies, etc.

  • Maintain high-quality client documentation and records management utilization electronic charting.

  • Monitor the care and services clients receive in residential setting.

  • Develop a positive working relationship with Residential Care manager/owner to meet needs of clients.

  • Develop and implement Individual Service Plan which consist of goals, resources, and interventions.

  • Review client’s progress every three months and revise Individual Service Plan as needed.

  • Review SUN-R 6 months after initial assessment and then at intervals of 12 months thereafter.

  • Revise client’s goals once met, or progress as they occur.

  • Assist mentally ill clients through crisis situations and/or arranging for the provision of such assistance by other professional or personal caregivers.

  • Perform incidental care to families of the mentally ill and assist those family members in understanding the mental illness.

  • Complete other duties as assigned.


Qualifications What we're looking for:


  • Bachelor's degree in social sciences field

  • Excellent 5-year driving record & reliable transportation

  • Excellent verbal and written communications skills

  • State of Alabama Adult Case Management Training

  • Knowledge of local community (Madison County Area)

  • Knowledge of local community agencies and resources

  • Certification in CPR and SAMA (or ability to obtain)

  • Crisis Prevention Intervention (CPI)
