Qualifications

• Bachelor’s degree preferred; 2 years of experience with vulnerable populations.
• Strong organizational, problem-solving, time management and analytical skills.
• Excellent verbal and written communication skills.
• Ability to multi-task and work effectively in a team; professional community representation.

Responsibilities

• Conduct strengths-based needs assessments with each family and develop measurable goals.
• Provide individualized home support including de-escalation, mediation, crisis support, and problem solving.
• Collaborate with local programs to connect participants with community resources and services.
• Complete documentation in HMIS and SharePoint per guidelines and procedures.
• Coordinate all financial requests with the Director of Family Services.
