Bowen Health in Fort Wayne, IN seeks a Skills Technician to assist families by advocating, teaching, and modeling daily living skills. The role may include in-home visits, transportation to appointments, and coordination with the Case Manager to plan services.

The position requires an Associate degree or 60 credits (or a HS diploma with 4 years of relevant experience). CPI, BLS and First Aid certifications, a valid driver’s license, and background checks are required.

Qualifications

• Associate degree or 60 undergraduate credits, or High School Diploma with 4 years of prior experience.
• Prefer two years in a mental health setting.
• CPI, BLS and First Aid certifications required after hire and kept current.
• Valid driver’s license and active auto insurance required.

Responsabilités

• Review and update patient intake information for accuracy.
• Coordinate homemaker services with Case Manager per care plan.
• Instruct patients in appropriate homemaker services and daily living skills.
• Advocate, teach, demonstrate, monitor, and model housekeeping skills.
• Transport patients to appointments and perform basic vehicle maintenance.
