Job summary

Family Promise Big Bend seeks a Family Care Manager to provide care management services for families living in the HOPE Community Family Shelter in Tallahassee, FL. The role involves engaging families to access housing, employment, income, health, transportation, childcare, documentation, and other services, while maintaining HMIS records and care plans.

The position requires a Bachelor’s in social work and at least two years in human services or related fields, with willingness to travel

Qualifications

• Bachelor’s degree in social work is required.
• Minimum of two years of experience in human services, case management, care management, housing services, homeless services, or related field.
• Experience working with families in a fast-paced environment.
• Knowledge of community resources and social service systems.

Responsibilities

• Provide fast-paced care management services to families residing in the shelter.
• Maintain accurate and timely client records, including HMIS documentation, service plans, case notes, assessments, referrals, and housing-related documentation.
• Engage families to access housing, employment, income, health, transportation, childcare, documentation, and other service needs.
