Job summary

Hope For Youth, Inc. in Town of Hempstead, NY is seeking a Case Planner for the HEART program to support children and families through home-based preventive services.

The role emphasizes frequent family contact, assessments, and coordinated case management within NYS policies. Minimum qualifications include a BSW or related bachelor’s with 2 years of casework or an MSW, strong written/verbal skills, and a valid NYS driver’s license.

Qualifications

• Excellent written, verbal and computer skills.
• Flexible schedule to accommodate clients and program.
• Valid NYS Driver's License in good standing.
• Read/write reports and log entries; ability to handle field duties.

Responsibilities

• Maintain regular contact with youth and families, including home visits and after-hours as needed.
• Complete initial and ongoing CANS-NY assessments and participate in school meetings/court appearances.
• Provide case management, life coaching, and parent skills training; make collateral contacts.
• Review service plans with youth and family; coordinate with supervisor on case records; complete documentation and paperwork.
• Attend staff meetings, treatment team meetings and trainings; perform other duties to ensure program success.
