Job summary

Hope For Youth, Inc. in Nassau County is seeking a Case Planner to support youth and families through case management, life coaching, and coordination with schools and courts.

This field-based role requires weekly contact, occasional evenings/weekends, and driving a personal vehicle. The ideal candidate has a BSW or MSW and 2 years of experience; bilingual Spanish/English is preferred.

Qualifications

• BSW or Bachelor's Degree in a related field with 2 years of casework experience
• MSW or Master’s degree in a related field
• Excellent verbal, written, and computer skills
• Ability to work a flexible, non-traditional schedule (evenings and weekends)
• Access to a reliable vehicle for regular home visits

Responsibilities

• Maintain weekly contact with youth and families, including evenings and weekends
• Provide case management, life coaching, and parent skills training
• Coordinate with schools, courts, and community partners
• Conduct CANS 2.0 assessments and document progress
• Attend school meetings and court appearances as needed
• Review and update service plans and discharge planning
