Job summary

Hope For Youth, Inc. seeks a Case Planner for the HEART program to support children and their families within a home-based preventive framework on Long Island. The role emphasizes sustaining families, coordinating services, and adherence to NYS and HFY guidelines.

The position offers comprehensive training opportunities and a benefits package, with typical hours totaling 35 per week and a nontraditional schedule to meet client needs.

Qualifications

• BSW (or Bachelor's in a related field) plus 2 years related casework experience or MSW.
• Excellent written, verbal and computer skills.
• Flexible schedule to accommodate clients and program.
• Valid NYS Driver's License in good standing.
• Physical requirements described (walking, driving, etc.).

Responsibilities

• Maintain regular contact with youth and families, in-home visits as required.
• Attend meetings, school and court appearances, discharge planning.
• Provide case management, life coaching and parent skill training.
• Review service plans with youth and family and document appropriately.
• Attend staff and treatment team meetings and trainings.
