Job summary

Hope For Youth, Inc. is seeking a Case Planner for the HEART program on Long Island. You will deliver case management, life coaching, and family support to prevent foster care disruption, while coordinating with schools, courts, and service providers.

The role requires a BSW (or related field) with 2 years of casework or an MSW and a valid NYS driver’s license. The 35-hour work week may include evenings and weekends, with strong training and advancement opportunities.

Qualifications

• Excellent written, verbal and computer skills.
• Flexible schedule to accommodate clients and program.
• Valid NYS Driver's License in good standing.
• Physical requirements: read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, driving own vehicle.

Responsibilities

• Maintain regular contact with youth and families, including home visits and evenings/weekends as needed.
• Complete initial and follow-up CANS-NY assessments; attend school meetings and court appearances.
• Provide case management, life coaching, and parent skill training; make collateral contacts as needed.
• Review service plans with youth and family; meet with supervisor to review case records.
