Job summary

Responsibilities include leading parenting education, documenting progress in the data system, and supporting outreach initiatives. The position requires driving to East Mesa locations and a Monday-to-Saturday schedule.

Qualifications

• Bachelor’s degree in education or related field required.
• Two years of relevant work experience required.
• Experience with children birth to 12 and families.
• CPR/First Aid certification and TB test required or obtainable.

Responsibilities

• Provide mid-level case management and service coordination for up to 25 families annually.
• Conduct family screenings, interviews, and assessments to identify needs and priorities.
• Develop individualized family service/action plans with families.
• Connect families to housing, food, parenting programs, healthcare, and mental health resources.
• Provide follow-up, track referrals, and monitor progress toward goals.
• Lead parenting education activities and family engagement efforts.
• Maintain accurate documentation in the data system and support audits.
• Engage in community outreach and build partnerships with service providers.
