Job summary

Homes for the Homeless is seeking a Client Care Coordinator in Staten Island to deliver mental health services to homeless families. You will conduct comprehensive assessments, coordinate social services, and collaborate with case management to help families achieve long-term goals.

Ideal candidates hold a Master’s in Social Work or Mental Health Counseling and a NY LMSW/LMHC license. Experience with trauma-informed care and knowledge of homelessness-related challenges are preferred.

Qualifications

• Must have a master's degree in Social Work or Mental Health Counseling.
• Must hold LMSW or LMHC in the state of New York.
• Familiarity with substance use disorders, trauma, domestic violence, and mental health conditions.
• Sensitivity and awareness of working with homeless and vulnerable populations.

Responsibilities

• Complete comprehensive bio-psychosocial assessments for families to identify strengths and service needs.
• Coordinate social services and counseling support with timely interventions.
• Ensure delivery of trauma-informed and family-centered care.
• Document progress toward service plan goals and outcomes.
• Provide full shelter support and refer families for counseling or other services when needed.
• Support the Family Services department alongside the Case Manager and staff.
• Improve linkages to mental health, education, and community services.
• Strengthen permanency outcomes for families with children in shelter.
• Manage records, data collection and evaluation of service effectiveness.
• Assist with unit inspections and conduct trainings for staff.

Education

Master's degree in Social Work or Mental Health Counseling

LMSW (Licensed Master Social Worker) or LMHC (Licensed Master Health Counselor) in New York
