Job summary

National Youth Advocate Program seeks an in-home advocate to partner with youth and families, delivering supportive services, transportation, parenting education, counseling, and resource referrals. You will collaborate with foster parents and community partners to help youth live safely and succeed in the community.

Responsibilities include traveling to engagements, documenting work, coordinating across mental health, medical, educational, and social supports, and participating in CQI

Qualifications

• Degree in Social Work or comparable Human Services field.
• Willingness to work flexible hours including evenings.
• Ability to be respectful and responsive to youth, families, and partners.
• Proficient use of desktop and mobile devices and common software.

Responsibilities

• Actively supports the mission, vision, and values of the organization.
• Provides in-home services, transportation, parenting education, counseling and referrals.
• Travels daily to deliver community-based services.
• Completes all required documentation promptly.
• Completes required trainings timely.
• Coordinates and monitors services across mental health, medical, educational, and social supports.
• Reviews and updates youth-based plans to ensure progress.
• Builds strong relationships with foster parents and families of origin.
• Assists in family reunification or stabilization per case plan.
• Provides safety assessment, crisis response, and behavioral stabilization as needed.
• Meets productivity thresholds consistently.
• Participates in Continuous Quality Improvement activities monthly/quarterly.
• Engages in weekly supervision process.
