Job summary

Danville-Pittsylvania Community Services seeks a Family Resource/Family Support Specialist to conduct screening, initial assessments, data entry, and documentation for the Healthy Families program. You will support home-based case management for families with young children and help ensure access to medical care and prevention services.

The role involves delivering presentations to referring agencies, complying with regulatory standards, and developing individualized service plans for at-risk

Qualifications

• Bachelor's Degree with at least 15 hours of coursework in Human Services and at least two years of experience with children five and under.
• Alternative degree in Psychology, Social Work, or related Human Services field, or certified psychiatric rehabilitation practitioner in Virginia.
• Valid driver's license and safe driving record; background checks and CANIS report as required.

Responsibilities

• Perform screening and initial assessment interviews.
• Enter initial data into the Program Management System for referred families.
• Enter assessments into the approved Medical Health Record database.
• Provide presentations to referring agencies in the community.
• Maintain documentation per Healthy Families America standards and Medicaid requirements.
• Provide intensive home-based case management to families with children at risk; develop and implement service plans.
