Job summary

Concord Hospital's Warner Practice is seeking a Clinical Practice Nurse 1 to provide high-quality care under the direction of practice providers. You will assess patients across the lifespan, plan and evaluate care, and collaborate with families and the health team.

The role requires an Associate Degree or Diploma in Nursing, NH licensure, and current BLS. You will triage calls, administer medications, document in the EMR, educate patients, and support a safe, efficient clinical workflow.

Qualifications

• Associate degree or diploma in nursing from an accredited program.
• New Hampshire nursing license required.
• Current BLS for Healthcare Providers or CPR for professionals.
• Experience interacting with a multi-disciplinary team.
• Ability to triage calls and manage patient flow.

Responsibilities

• Establishes priorities in organizing daily activities and maintains efficient patient flow.
• Triages patient phone calls following guidelines to identify health problems or concerns.
• Administers prescribed medications.
• Provides education to patients in response to identified needs.
• Documents health information in the patient EMR.
• Delegates care using the five rights and provides feedback.
• Applies nursing process to make clinical judgments and decisions.
• Completes practice-specific clinical skill competencies annually.
