Job summary

Concord Hospital Health System is seeking a Clinical Practice Nurse 1 to deliver high quality patient care including assessment, treatment, planning, and care evaluation. This position requires NH nursing licensure, an associate degree or diploma, and current BLS/CPR certification, with collaboration across a multidisciplinary team.

You will triage calls, administer medications, educate patients, document in the EMR, and uphold nursing process with ongoing skill competencies.

Qualifications

• Associate degree or diploma in nursing from an accredited school.
• New Hampshire Board of Nursing licensure required.
• Current BLS for Healthcare Providers or CPR for professionals required.
• Experience interacting with a multidisciplinary staff and patients.
• Ability to triage patient calls and provide quality care.
• Medication administration and patient education responsibilities.
• Documentation in the electronic medical record.
• Ability to delegate using the five rights and apply nursing process.
• Annual completion of clinical skill competencies.

Responsibilities

• Establishes priorities in organizing daily activities and maintains efficient patient flow.
• Triages patient phone calls following guidelines to identify health problems.
• Administers prescribed medications.
• Provides education to patients based on identified needs.
