The assignment involves working weekdays from 8:30 AM to 4:20 PM, managing a diverse caseload of geriatric patients across high-risk, emerging risk, and low-risk categories. This role requires excellent coding and charting skills, as well as a patient-centered approach to care. • Conduct thorough patient examinations and develop personalized care plans
• Accurately code and chart patient encounters using eClinicalWorks EHR system
• Collaborate with other healthcare professionals to ensure coordinated care
• Stay updated on the latest geriatric care practices and treatment options
• Educate patients and their families on health management and preventive measures
• Participate in care team meetings and contribute to quality improvement initiatives
