Established in 1980, the Greater Lawrence Family Health Center (GLFHC) is a multi-site, mission-driven non-profit organization employing over 700 staff members. Its primary focus is providing high-quality patient care to residents throughout the Merrimack Valley. Recognized nationally as a leader in community medicine—including family practice, pediatrics, internal medicine, and geriatrics—GLFHC operates clinical sites across the service area and sponsors the Lawrence Family Medicine Residency program.
The Family Partner is responsible for addressing behavioral health (BH) and high-risk social needs (HRSN) of children, youth, and families attributed to the ACO population at the Health Center. This role involves serving as a patient and family advocate, acting as a primary liaison and point of contact for ACO patients enrolled in CBHI, Family Resource Centers, and educational settings. The Family Partner collaborates with internal stakeholders such as care managers, primary care clinicians, and integrated care teams. The position requires on-site presence at least monthly, with availability for virtual engagement with patients, families, and external service providers. The Family Partner provides expertise to engage families, address challenges, and remove barriers, including coaching parents on navigating children's mental health systems.
