Job Description

The Family Partner provides Family Support and Training (Caregiver Peer-to-Peer Support) under the supervision of the Family Partner Supervisor. This role involves establishing a structured, strength-based, one-to-one relationship with parents/caregivers to support youth with emotional and behavioral challenges. The goal is to improve parental capacity to support their youth, facilitating youth’s return or ongoing participation in the community. Activities include education, system navigation assistance, fostering empowerment, resource linkage, and coaching, aimed at achieving individual child/youth goals.

  1. Assist in creating a positive, collaborative environment where all employees, youth, and families feel welcomed and supported.
  2. Deliver services at offices, in the community, or in the family’s home based on preferences.
  3. Build strong, functional relationships with enrolled youth and their families.
  4. Develop outreach strategies to enable full participation in the Wraparound Process.
  5. Assist in developing Care Planning Teams with clinical providers, family, and other supports.
  6. Mentor parents/caregivers to advocate effectively and navigate human services systems.
  7. Work closely with the Care Planning Team to monitor and advocate for the family’s needs.
  8. Participate in meetings, training, supervision, and quality activities as directed.
Position Qualifications
Family Partner I
  • High School Diploma/GED/HiSet
  • Satisfactory CORI background check
  • Valid US driver's license and insured vehicle for work-related transportation
  • Knowledge of system of care values: Child Centered, Family Driven, Strength-Based, Culturally Responsive, Collaborative, and Continuous Quality Improvement
  • Clear boundaries
  • Minimum 1 year experience working with youth/families
  • Experience as a caregiver of a youth with special needs
Family Partner II
  • Bachelor’s Degree in psychology, counseling, or social work
  • Satisfactory CORI background check
  • Valid US driver's license and insured vehicle for work-related transportation
  • Knowledge of system of care values as above
  • Clear boundaries
  • Minimum 1 year experience working with youth/families
  • Experience as a caregiver of a youth with special needs
