What We’re Looking For
Trillium Health Resources has a career opening for a Community SOC Family Partner to join our Team! The Community SOC Family Partner is responsible for educating parents and caregivers about how to effectively navigate the child-serving physical, behavioral and social service systems for themselves and about the existence of informal community resources available. In this role you will provide a system of care for young people and families receiving child mental health and substance use services for assigned counties in the Trillium Catchment Area by ensuring members receive whole person care focused on all members’ needs including physical health, behavioral health, and social determinants of health that helps to establish positive outcomes for members and transform their lives for the better. Qualifications:
Education/Experience:
High School Diploma or GED is required with a minimum of four (4) years of experience as a primary caregiver for a child or youth receiving Medicaid or State funded BH services.
Preferred Experience:
Two (2) years paid experience working directly with individuals with BH/MH or IDD conditions. Paid experience related to the counties in which they are applying.
License/Certification:
Must have a valid driver’s license
Preferred License/Certification:
Certified Peer Support Specialist preferred.
Location:
Remote within Region.
Must reside in NC to be considered for remote status.
Must be able to travel to a Trillium office location and within catchment as required.
