• Develops and maintains a community resource network to link families with community supports.
• Maintains and updates family caseload and visit-hour goals.
• Maintains confidentiality of families and services.

Responsibilities

• Plans and facilitates family and parent group connections to build relationships and support parent learning.
• Plans visits using Visit Tracker, conducts visits in home, center, or via virtual platform (e.g. Google Meet).
• Responds to inquiries about program requirements and procedures to ensure consistent implementation.
• Responds to emergencies encountered during home visits to ensure safety.
