Job summary

A-Line Staffing is seeking a Family Nurse Practitioner for Primary Care in Crystal Lake, IL. The role provides comprehensive care to patients 18 months and older in a primary care setting with a schedule of 3–4 days per week and mandatory weekend coverage.

The position offers compensation at $75.00–$78.00 per hour with benefits for full-time employees, including a 401(k) plan with company match. EPIC electronic medical records are used for documentation.

Qualifications

• FNP license and 1+ year experience in primary care or related fields.
• Experience in urgent care, ER, internal/family medicine or hospital settings is valued.

Responsibilities

• Provide comprehensive primary care to patients 18 months and older.
• Perform advanced assessments, diagnose conditions, and develop treatment plans.
• Prescribe medications and order/interpret diagnostic tests per guidelines.
• Manage common conditions including infections, UTIs, dermatologic issues, allergies and minor injuries.
• Deliver preventive care including vaccinations, screenings, and routine exams.
• Conduct women’s health evaluations and refer to OB/GYN as needed.
• Monitor chronic diseases such as hypertension, diabetes, and asthma.
• Educate patients on disease management and prevention strategies.
• Document encounters in EPIC and ensure regulatory compliance.
• Collaborate with physicians and interdisciplinary teams for continuity of care.

Family Nurse Practitioner - Primary Care Responsibilities

• Provide comprehensive, evidence-based primary care to patients 18 months of age and older in a primary care setting
• Perform advanced patient assessments, diagnose acute and chronic conditions, and develop treatment plans
• Prescribe medications and order/interpreting diagnostic tests in accordance with state licensure and clinical guidelines
• Manage common conditions including respiratory infections, UTIs, dermatologic conditions, allergies, and minor injuries
• Deliver preventive care services including vaccinations, health screenings, and routine physical examinations
• Conduct women’s health evaluations including pregnancy testing and STI screening; refer to OB/GYN for specialized care as needed
• Monitor and manage chronic diseases such as hypertension, diabetes, and asthma
• Provide patient education on disease management, medications, and preventive health strategies
• Document all patient encounters in EPIC - electronic medical records ensuring compliance with regulatory and quality standards
• Collaborate with physicians and interdisciplinary teams to ensure continuity of care and appropriate referrals

Family Nurse Practitioner - Primary Care Requirements

• FNP License 1 + years of experience of primary care, urgent care, ER, internal/family medicine or hospital experience INDKS
