Job summary

The HealthCare Connection is seeking a Family Nurse Practitioner to join our clinical team in Ohio, delivering primary care across a spectrum of conditions and ages. You will collaborate with physicians to provide patient-centered care and optimize health outcomes in a community-focused FQHC setting.

Candidates should have an NP credential, DEA license, and at least 5 years in family practice, with a preference for those who enjoy pediatric care and ambulatory settings.

Qualifications

• Master’s or Doctoral degree in Nursing.
• Certified Nurse Practitioner licensed in the State of Ohio.
• Minimum of 5 years of experience in family medical practice.
• FNP preferred, with a strong desire to work with children.
• Experience in a physician office, ambulatory care, or clinical health setting preferred.

Responsibilities

• Provide high-quality patient care through diagnosis and treatment of acute and chronic conditions.
• Obtain and review patient histories and perform comprehensive physical exams.
• Order, perform, and interpret diagnostic tests and preventive screenings.
• Develop and manage treatment plans, including prescribing medications and educating patients.
• Promote preventive care through immunizations, screenings, and patient education.
