Job summary

Sentara Health in Harrisonburg, VA is seeking a Family Nurse Practitioner to join our Community Care team delivering primary care at mobile clinics and designated sites for underserved populations.

You will co-manage patients with a diverse team, participate in care at schools, mobile RV locations, and collaborate with RN, counselors, and care management; typical schedule Monday–Friday with occasional Saturdays. Competitive base pay with incentives.

Qualifications

• 1+ year APP experience required.
• Spanish language skills are helpful but not required.
• Experience in FQHC/Community Health Center beneficial.
• Post-master's training in Community Health Care preferred.

Responsibilities

• Provide direct primary care services to children and adults in underserved communities.
• Collaborate with RN, Counselors, Care Management and other team members to improve patients’ health holistically.
• Share call schedule and review quality and outcome metrics monthly.
• Partner with City Schools for sports physicals and immunizations.
• Work from Mobile Care RV at designated locations (Hope Distributed, Navigation Center).
