Job summary

Sentara Health Plans seeks a compassionate Family Nurse Practitioner to join the Mobile Community Care team in Norfolk, VA and surrounding areas. The role involves direct primary care from a Mobile Care RV, serving children and adults in underserved communities.

Part-time (20 hours/0.5 FTE) with shared call, collaboration with a diverse care team, and opportunities to engage with local civic leaders. Epic EHR experience is preferred.

Qualifications

• 1+ year APP experience required.
• Experience in FQHC or Community Health Center is a plus.
• Post-master’s training in Community Health Care is beneficial.

Responsibilities

• Provide primary care services via mobile health units to children and adults.
• Treat family medicine, pediatrics, and adults with complex issues.
• Co-manage patients with a diverse care team and participate in community education.
