Sentara Health is seeking a Family Nurse Practitioner to join our Community Care team in Norfolk, VA. The position operates 20 hours per week (0.5 FTE) from a Mobile Care RV, delivering primary care to children and adults in underserved communities across Hampton Roads and Northeast North Carolina.

You will co-manage patients with a diverse care team, treat family medicine, pediatrics, and adults with complex issues, and review quality metrics monthly.

Qualifications

• 1+ year APP experience required.
• Experience in FQHC or Community Health Center, or post-masters training in Community Health Care would be beneficial.

Responsibilities

• Work from Mobile Care RV delivering direct primary care to children and adults in underserved communities.
• Treat family medicine, pediatrics, and adults with complex issues.
• Co-manage patients with RN, counselors, care management, and other team members to improve patients' health.
