Job summary

Sentara Health is seeking a compassionate Family Nurse Practitioner to join its Community Care Team in Richmond, VA. You will provide primary care to children and adults via clinic visits and Mobile Care RV, serving uninsured and Medicaid populations.

You'll collaborate with RNs, counselors, and care managers, participate in quality reviews, and work with City Schools on sports physicals and health screenings. Bilingual skills are a plus.

Qualifications

• 1+ year APP experience required.
• Spanish language helpful but not required.
• Experience in primary care and community health settings preferred.
• FQHC or community health center experience is beneficial.

Responsibilities

• Provide primary care services to children and adults in clinic and Mobile Care RV.
• Collaborate with RN, counselors, care management and staff to holistically manage patients.
• Review quality and outcome metrics monthly with the care team.
• Partner with City Schools to provide sports physicals, immunizations and screenings.

Location and Schedule

City/State: Richmond, VA. Work Shift: First (Days). Provider Specialty: Community Care.

Position Highlights

• Location: 5116 Richmond Henrico Turnpike, Richmond VA 23227 and surrounding area (within 1 hour)
• Schedule: Monday – Friday plus 1 Saturdays per month.
• Provide primary care services to children and adults through both the clinic and Mobile Care RV, bringing healthcare directly to underserved communities throughout the Richmond region and surrounding areas within a one‑hour radius.
• Partner with City Schools providing sports physicals, immunizations, health screenings, community events, etc.
• Shared call schedule, with RN first call
• Co‑manage patients with a diverse care team to include, but not limited to, RN, Counselors, Care Management, and other team members to improve our patients’ health holistically.
• Review quality and outcome metrics monthly with the healthcare team
• Collaborate with local civic and faith‑based leaders on community education programs
• Experience working in a FQHC or Community Health Center, or post‑masters training in Community Health Care would be of great benefit to the practice
• 1+ year APP experience required
• The ability to speak Spanish is extremely helpful in this role, but not required

We provide market‑competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full‑time employment is $100,900.80 - $312,000.00 annually. Additional compensation may be available for this role, such as shift differentials, standby/on‑call pay, overtime, shift premiums, extra‑shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. Note: If an annual salary is posted, it is based on a full‑time colleague working 2,080 hours annually.
