Copa Health is seeking a compassionate Family Nurse Practitioner to join our integrated primary care team serving the West Valley Phoenix metropolitan area. This full-time role provides patient-centered care in an integrated behavioral health setting, with a focus on diverse member outcomes.

You will deliver comprehensive care, collaborate with behavioral health and nursing staff, document in the EHR, and participate in quality initiatives to improve outcomes.

Qualifications

• Current and valid FNP license in Arizona.
• National certification as a Family Nurse Practitioner.
• DEA license or ability to obtain upon hire.

Responsabilités

• Provide comprehensive primary care across the lifespan.
• Manage chronic and acute conditions with the behavioral health team.
• Document evaluations, medications, and follow-ups in the EHR.
• Participate in integrated team meetings and performance initiatives.
• Promote whole-person wellness with education and prevention.
