Job summary

Copa Health Inc. in the West Valley Phoenix metropolitan area seeks a compassionate Family Nurse Practitioner to join our integrated primary care team in an outpatient behavioral health setting.

You will provide comprehensive primary care across the lifespan, manage chronic and acute conditions with a multidisciplinary team, perform evaluations and medication management, maintain timely EHR documentation, and participate in quality initiatives.

Qualifications

• Current and valid FNP license in Arizona.
• National certification as Family Nurse Practitioner.
• DEA license (or ability to obtain upon hire).
• Experience in outpatient or primary care settings preferred; behavioral health experience a plus.

Responsibilities

• Provide comprehensive primary care, including assessment, diagnosis, and treatment of members across the lifespan.
• Manage chronic and acute conditions in collaboration with behavioral health providers, nursing staff, and case managers.
• Complete evaluations, medication management, and follow-up care in accordance with best practice standards.
