Job summary

Southwest Behavioral & Health Services seeks a Part Time Family Nurse Practitioner or Physician Assistant to join the Metro Outpatient Clinic in North Phoenix. Schedule is 16-20 hours weekly, Tue and Thu in-office. Provide primary care for physical and mental health needs in a community health setting, with a team-based approach.

Ideal candidate is licensed in Arizona, with APRN certification and DEA, and experienced in primary care or pediatric/adolescent care; fingerprint clearance is required.

Qualifications

• Master's degree in nursing and APRN certification as FNP/ANP/PA Pediatric NP.
• Arizona license and valid DEA certificate required.
• Experience as an APRN in primary care; pediatric/adolescent sites preferred.
• Fingerprint clearance by AZ DPS or eligible to obtain on hire.
• Credentialed/privileged with RBHA and local medical facilities.

Responsibilities

• Provide health maintenance, assessment, diagnosis, and treatment of acute and chronic conditions.
• Order and interpret diagnostic tests; refer to medical specialists as needed.
• Educate patients about preventive care, medications, and treatments.
• Work collaboratively with SBH/RBHA teams and other primary care providers.
• Attend required SBH and RBHA meetings.
