Premise Health Inc. is seeking a full-time Nurse Practitioner for our Health Center in Pleasanton, CA. Schedule is Monday–Friday, 8:00am–5:00pm, with a $5,000 sign-on bonus. You will conduct exams, order tests, develop care plans, and educate patients to promote wellness.

You must hold NP licensure, national certification, and applicable prescriptive authority. DEA license may be required. Minimum 3 years of ambulatory or occupational health experience is expected.

Qualifications

• State RN license to practice is required.
• Master’s Degree in Nursing is required (or transcripts).
• Current national certification (ANCC or AANP) is required.
• Nurse Practitioner license and prescriptive authority as applicable.
• DEA license and state controlled substance authority if required.

As a Full-Time Nurse Practitioner (NP), you'll provide care to client employees and their dependents in our Health Center located in Pleasanton, CA. The scheduled hours are Monday-Friday 8:00am-5:00pm.

What You’ll Do

• Conducts physical exams and preventive health measures per guidelines
• Performs procedures such as injections, suturing, wound care, and infection management
• Develops patient care plans, records progress, and ensures continuity of care
• Educates patients on treatment plans, health maintenance, and development
• Orders and interprets diagnostic tests to assess clinical conditions
• Collaborates with physicians and healthcare teams for comprehensive care
• Prescribes or recommends treatments, including medications and therapies
• Refers patients to specialists or physicians as needed
• Maintains patient health records ensure accurate and up-to-date documentation and records
• Educates and coaches nursing staff on best practices
• Performs other duties as assigned

What You’ll Bring

• Maintains state RN licenses/certificates to practice
• Maintains Nurse Practitioner licenses/certificates to practice
• Maintains State Prescriptive Authority (if applicable)
• Federal DEA license (matching each state of practice as applicable)
• State Narcotic/Controlled Substance license if required in your state (Alabama, Connecticut, DC, Delaware, Hawaii, Idaho, Illinois, Indiana, Iowa, Louisiana, Maryland, Massachusetts, Michigan, Nevada, New Jersey, New Mexico, Oklahoma, Rhode Island, South Carolina, South Dakota, Utah, Wyoming, Puerto Rico) (New York ROPES registration)
• Current national certification (ANCC or AANP required)
• Master’s Degree in Science of Nursing (if Master Degree in Science certificate does not state Nursing on it then a copy of transcripts is required)
• Current certification with hands-on training in AHA, ARC or equivalent Basic Life Support for health care providers is required; Advanced Cardiac Life Support may also be required based on contract scope of services
• At least three years of clinical experience as a Nurse Practitioner in an ambulatory care, occupational health, family practice or emergency department setting

Premise provides its reasonable and genuinely expected range of compensation for this job of $165,000 - $175,000.00 annually. A number of factors will influence the rate offered for this position, including your experience, qualifications, geographic location, and other factors.
