SGS Consulting is seeking a Family Nurse Practitioner to provide comprehensive primary care in a clinic setting in Milton, MA. You will assess, diagnose, treat, and follow up on acute and chronic conditions, while developing personalized care plans with strong patient engagement.

The role emphasizes preventive care education, collaboration with healthcare teams, and adaptable scheduling, including weekends or holidays as needed. A current FNP certification and active APRN license are required.

Qualifications

• Master’s degree in Family Nurse Practitioner program required.
• Current National Board Certification (FNP-C or FNP-BC) required.
• Active state license to practice as an Advanced Practice Registered Nurse.
• BLS certification required upon offer.
• Eligible or willing to obtain DEA registration.
• Ability to work independently and in collaborative care environments.

Responsibilities

• Provide comprehensive patient care, including assessment, diagnosis, treatment, and follow-up for acute and chronic conditions.
• Conduct health counseling and develop personalized care plans.
• Perform routine examinations and diagnostic testing.
• Educate patients on preventive care, lifestyle choices, and chronic condition management.
• Coordinate with other healthcare professionals to ensure quality, integrated care.
• Manage administrative duties: opening/closing clinic, maintaining inventory, verifying insurance, processing payments, and ensuring a clean clinical environment.
• Support wellness and behavioral health initiatives to improve patient outcomes.
• Work flexible hours, including alternate weekends and possible holiday rotations.
