Qualifikationen

• 1+ year APP experience required
• Spanish ability helpful but not required
• Experience in FQHC or community health center preferred
• Post-master's training in Community Health Care would be beneficial

Aufgaben

• Provide primary care to children and adults in clinic and Mobile Care RV
• Collaborate with diverse care team including RN, counselors, and care management
• Partner with City Schools for sports physicals, immunizations, screenings and events
• Review quality and outcomes metrics monthly with the healthcare team
• Engage with local civic and faith leaders on community education programs
• Share call schedule with RN on call and support team-based care
