Sentara Health's Community Care Team in Harrisonburg, VA is seeking a compassionate Family Nurse Practitioner to deliver primary care in community clinics and mobile units serving uninsured and Medicaid populations. You will co-manage patients with an integrated team and participate in quality improvement and community education.

The schedule is Monday–Friday 9:30–6:30 with 1–2 Saturdays monthly; work from Mobile Care RV at Hope Distributed and Navigation Center sites, with Epic EHR support and

Qualifications

• 1+ year APP experience required.
• Experience in FQHC or Community Health Center is a plus.
• Spanish language skills are helpful but not required.

Responsibilities

• Co-manage patients with a diverse care team (RN, counselors, care management).
• Provide primary care services at community clinics and Mobile Care RV units.
• Partner with local schools for sports physicals and immunizations.
• Review quality and outcome metrics monthly with the team.
