Job summary

Sentara Healthcare, Inc. seeks a compassionate Family Nurse Practitioner for Part-Time roles in the Hampton Roads VA and Northeast North Carolina regions. You will work 20 hours per week from a Mobile Care RV, delivering primary care to children and adults in underserved communities.

Collaborate with a diverse team to manage family medicine, pediatrics, and complex adult cases. Eligibility for professional growth and community outreach is highlighted.

Qualifications

• 1+ year APP experience required.
• Experience in an FQHC or community health center is a plus.
• Co-manage patients with a diverse care team.
• Licensed to practice as a Family Nurse Practitioner (FNP).
• Experience with mobile health care delivery is beneficial.

Responsibilities

• Provide direct primary care to children and adults via a Mobile Care RV.
• Treat family medicine, pediatrics, and adults with complex issues.
• Collaborate with RN, counselors, care management and other team members.
• Review quality and outcome metrics monthly with the healthcare team.
• Collaborate with civic and faith-based leaders on community education programs.
