Mannacare Health and Wellness Medical Center in Lima, Ohio is seeking a licensed Nurse Practitioner to join our primary care team. The role delivers comprehensive primary care across the lifespan, including routine exams, chronic disease management, preventive care, and treatment of acute conditions, in collaboration with our clinical and administrative teams.

The ideal candidate holds an Ohio APRN license with FNP, AGPCNP, or ANP credentials, PA-C with primary care experience, DEA registration,

Qualifications

• Active Ohio APRN license with Family Nurse Practitioner (FNP) certification
• Active Ohio APRN license with Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) certification
• Active Ohio APRN license with Adult Nurse Practitioner (ANP) certification
• Active Ohio PA-C license with primary care experience
• Active DEA registration or ability to obtain one
• Experience with Tebra or comparable EHR/practice management software
• Experience in a primary care or family medicine setting
• Strong communication skills and comfort working within a collaborative, multi-provider practice

Responsibilities

• Provide comprehensive primary care services to patients across the lifespan.
• Diagnose and treat acute conditions in a primary care setting.
• Manage chronic diseases through preventive care and ongoing monitoring.
