Qualifikationen

• Master's Degree and current NP license (Family Nurse Practitioner required) with three years of practice serving gynecology, obstetrics and pediatric patients.
• CPR and DEA certification.
• Experience serving underserved populations.
• Experience with trauma, housing insecurity, or related social determinants of health.
• Strong communication, clinical and organizational skills.
• Experience with EHR systems such as eClinicalWorks.

Aufgaben

Salary Range:$113,775 - $117,757 /yr based on experience

Project HOME is seeking a Family Nurse Practitioner to join our integrated healthcare team and help eliminate health disparities for individuals and families experiencing homelessness, housing insecurity, and systemic barriers to care. As a Family Nurse Practitioner, you will deliver primary care services to children, adults, and OB/GYN patients.

The hours are Monday-Friday, 8:00 AM-4:00 PM located at the Stephen Klein Wellness Center at 2144 Cecil B. Moore Ave, Philadelphia, PA 19121.

What You'll Do:

As a key member of our community-based care team, you will:

• See 9 patients per session, 8 sessions per week, with a focus on holistic and trauma-informed care
• Collaborate with a dedicated team of medical assistants, behavioral health consultants, care managers, and peer specialists
• Address both clinical and social determinants of health, recognizing the unique needs of marginalized populations
• Uphold our mission and values through ethical, equitable, and evidence-based care
• Participate in quality improvement initiatives, staff development, and clinical leadership opportunities
• Mentor students (NP, PA, and MD) and engage in ongoing CME for professional growth

What We're Looking For

• Master's Degree and current NP license (Family Nurse Practitioner required) with three (3) years of practice experience serving gynecology, obstetrics, and pediatric patients
• CPR and DEA certification
• Experience or strong interest in serving underserved and vulnerable populations
• Comfort working with patients experiencing trauma, behavioral health concerns, or housing insecurity
• Strong communication, clinical, and organizational skills
• Experience with eClinicalWorks or similar EHR systems preferred
• Light lifting, standing, and walking throughout the day as needed
